Rochelle Nursing Home Fined $25,000 After State Finds Care Failures

SPRINGFIELD — Rochelle Gardens Care Center was fined $25,000 after a state investigation found the facility failed to follow care protocols when a resident fell in November 2024. The Illinois Department of Public Health cited the Rochelle facility with a Type A violation, noting staff did not properly assess the resident, notify a physician, or monitor the resident for signs of injury over a 72-hour period as required. The delay led to a fractured hip going untreated for two days. The fine was doubled due to the violation’s high-risk designation. Rochelle Gardens was among the facilities listed in IDPH’s first-quarter 2025 nursing home violation report.
